How did you find out about MetroMalts?:

Why do you want to foster a Maltese or other toy breed?:

 YOUR HOUSEHOLD:

  • Does your landlord, co-op or condo permit pets?
    Yes
    No
  • If yes, provide the name and phone # of your landlord or managing agent to verify:

    Landlord/managing agent:

    Phone #:

  • How many people live at your residence?
  • List ages and relationship to applicant:

Please note:  The Maltese breed is not a good choice for families with children under the
age of 10 (or for those planning to start a family) and placement in such a home is against
MetroMalts policy.  While we appreciate your interest in a rescue Maltese, please choose
a less fragile breed for your family.

  • Do you or any members of your household have any medical condition
(including allergies) that might interfere with fostering a dog?:
    Yes
    No
  • If yes, please explain:

Please be aware that while some people are less allergic to Maltese than other breeds, there is
no guarantee that owning a Maltese will not result in an allergic reaction.

    YOUR LIFESTYLE:

  • Do all adults in your household work full-time?:
    Yes
    No
  • If yes, what is the maximum number of hours at a time your dog will be alone?:

  • Where will your dog be kept when alone?:
  • Who will be the primary caretaker of the dog?:
  •

  • Have you ever fostered a dog before?

Yes
No

If yes, describe your experience as a foster.

Thank you for taking the time to complete this questionnaire.   It is an important part of the foster
process. Before you submit this application, you need to be aware of the following:

HOME VISIT AND ATTACHMENTS:

Each prospective foster home will be visited by a Metropolitan Maltese Rescue placement volunteer
to evaluate its qualifications.

FOSTER AGREEMENT:

After your application is approved and before a dog is placed in your home, you will be asked to sign
a foster agreement which will include, among other things, a statement that you will return the dog
to Metropolitan Maltese Rescue if at any time or for any reason you are no longer willing or able to
keep the dog in your home.
